Job summary

Proforma Health Inc. seeks a licensed Physical Therapist to evaluate, diagnose, and treat patients to improve mobility, reduce pain, and enhance quality of life across multiple healthcare settings.

You will develop evidence-based, personalized treatment plans, operate and maintain equipment, assess progress, and educate patients and caregivers on home exercises and injury prevention while ensuring safety and quality care.

Qualifications

• Valid Physical Therapy license compliant with state regulations.
• Current BLS certification.
• Strong clinical skills, critical thinking, and communication abilities.
• Proficient in English.
• Ability to operate and maintain specialized equipment and assist patients.
• Capable of working under pressure with attention to detail and irregular schedules.
