Job summary

Healthforce is seeking a General Physical Therapist for a 13-week travel assignment in Clearlake, California. The role runs 36 hours per week, day shift, Mon–Fri, with a start date of 10/12/2026.

You will evaluate patients, develop treatment plans, document progress, and collaborate with a multidisciplinary team. Must hold a DPT and California license or eligibility, CPR/BLS, and have home health experience.

Qualifications

• DPT degree required; CA licensure or eligibility.
• CPR/BLS certification required.
• Minimum 1 year of home health experience.
• Valid driver's license and at least 21 years old.
• Knowledge of HCHB and OASIS; EPIC experience preferred.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans to restore and optimize physical function.
• Provide physical therapy interventions using appropriate techniques and protocols; adjust plans based on progress.
• Document assessments, treatment plans, progress notes, and discharge plans within established deadlines.
• Collaborate with an interdisciplinary team and participate in group or patient education sessions as needed.
• Supervise assigned assistants and coordinate care delivery within the patient’s care plan.
• Support discharge planning and communicate plans to the patient and care team.
