Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel job in Puyallup, Washington. The assignment runs 13 weeks, with 40 hours per week and an 8-hour shift.

Travel throughout the service area is required, with potential holidays and weekend rotations as needed. Minimum requirements include a PT degree from an accredited program, an active WA license (or compact licensure), BLS/CPR as applicable, a current CPR certification, and a valid driver's license with

Qualifications

• PT degree from accredited programs approved by APTA
• Active WA State license or compact license
• BLS/CPR certification as applicable
• Current Physical Therapy licensure in WA or compact
• Current CPR certification
• Valid driver's license and vehicle insurance

Responsibilities

• Provide home health physical therapy services to patients
• Travel across service area as assigned
• Coordinate care with healthcare team
• Document patient progress and maintain records
• Participate in care planning and discharge planning
