Continuum Therapy Partners is seeking a Physical Therapist (PT) for a 13-week travel assignment to support Heritage Manor Health Center and Guardian Angels Health & Rehab in the Chisholm/Hibbing, MN area. This full-time travel role offers competitive weekly pay and direct support from a team that understands the clinical environment.

Qualifications

• Active Minnesota Physical Therapist / PT license or eligibility.
• Experience in skilled nursing/post-acute rehab.
• Travel experience preferred but not required.
• Experience with Net Health is a plus.
• Strong documentation, communication, and teamwork skills.
• Ability to start quickly and complete onboarding before assignment start.

Responsibilities

• Provide skilled Physical Therapy services per standards, orders, and care plans.
• Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.
• Develop and update individualized treatment plans based on patient needs and progress.
• Collaborate with rehab leadership, nursing, facility teams, families, and care partners.
• Support discharge planning, patient education, and functional outcomes.
• Approach the assignment with professionalism, flexibility, and patient-centered care.
