Job summary

Ardor Health Solutions seeks a Travel Physical Therapist for a 13-week SNF assignment in Fall River, Massachusetts. This role offers full-time hours working with adult and geriatric patients as part of a collaborative rehab team.

You will evaluate, develop and adjust treatment plans, deliver gait and balance training, and coordinate with nursing, physicians, OT, SLP and case management; weekly pay and comprehensive benefits are provided.

Qualifications

• Massachusetts PT license required.
• BLS certification required.
• SNF or post-acute experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on each patient’s goals.
• Treat orthopedic, neurological, post-surgical, and complex patients.
• Provide gait, balance, therapeutic exercise and mobility training.
• Collaborate with nursing, physicians, OT, SLP and case management.
• Educate patients and caregivers on safety strategies and home exercise.
• Complete timely documentation and progress reporting.
