Qualifications

• Must have graduated from an accredited school.
• Current state license or actively in process for licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.

Responsibilities

• Deliver PT services in SNF/LTC settings.
• Create and implement patient care plans.
• Coordinate with interdisciplinary teams to meet treatment goals.
