Job summary

Centra Healthcare Allied seeks a Travel Physical Therapist for a 13-week contract in Cheyenne, Wyoming. The SNF/LTC assignment requires 40 hours per week with an 8-hour shift and ASAP start. This travel role supports ongoing patient care across facilities.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
