Job summary

Northshore Home Health Care seeks a Travel Physical Therapist for a contract role in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting. You will develop and implement individualized care plans and provide skilled PT interventions per physician orders.

The ideal candidate will hold an accredited physical therapy degree, have active licensure or an active licensure application, and possess CPR certification with recent PT experience or graduation.
