Responsibilities

• Evaluate residents for PT needs and develop plans of care to improve mobility, strength, balance, and independence.
• Implement PT interventions: gait training, ROM, endurance, and task-specific training in SNF setting.
• Assess progress, document outcomes, and adjust plans in collaboration with care team.
• Educate residents and families on home exercise programs, safety, fall prevention, and activity modification.
• Communicate with nurses, physicians, and other therapists to ensure cohesive, patient-centered care and timely discharge planning.
• Maintain accurate documentation in EHR and comply with regulatory standards.
