Physical Therapist – Skilled Nursing Facility in Waterville, ME, US. This travel temporary assignment starts on 10/19/2026 and runs for 13 weeks. Responsibilities

• Evaluate residents for physical therapy needs and develop individualized plans of care aimed at improving mobility, strength, balance, and functional independence
• Implement evidence-based PT interventions, including gait training, ROM exercises, endurance activities, and task-specific training within a skilled nursing setting
• Assess and monitor progress, document outcomes, and adjust treatment plans in collaboration with the care team
• Educate residents and families on home exercise programs, safety, fall prevention, and activity modification to support continued progress
• Communicate clearly with nurses, physicians, and other therapists to ensure cohesive, patient-centered care and timely discharge planning
• Maintain accurate documentation in the electronic health record and ensure compliance with regulatory and facility standards

Requirements

• Active Physical Therapist license (state of Maine or eligible for licensure by endorsement)
• Ability to work in a skilled nursing facility setting and collaborate within an interdisciplinary team
• Experience in geriatrics or SNF rehabilitation preferred
• Strong documentation skills and proficiency with electronic health records
