Job summary

Healthforce in Monterey, California is seeking a Physical Therapist for a travel assignment at a skilled nursing facility. The position runs for 13 weeks starting 11/23/2026, with guaranteed hours and a 5x8 daytime shift from 08:00 to 16:30.

Weekly pay ranges from $2,567 to $2,715, with 36 hours guaranteed per week. You will evaluate residents, develop goals, provide interventions, document outcomes, and coordinate with an interdisciplinary team.

Qualifications

• Active California PT license is required.
• Experience in skilled nursing or geriatric rehab preferred.
• Ability to travel and meet credentialing requirements.
• Strong documentation and communication skills.

Responsibilities

• Conduct initial evaluations to determine PT needs in SNF residents.
• Develop and implement individualized treatment plans with measurable goals.
• Provide evidence-based interventions (exercises, gait/balance, mobility, manual therapy).
• Reassess progress and adjust plans to maximize recovery.
• Collaborate with nursing, OT, speech, and physicians to coordinate care.
• Educate patients and families on home exercise programs and safety.
