Qualifications

• Active physical therapist license in New Hampshire in good standing.
• Doctor of Physical Therapy (DPT) degree.
• CPR/BLS certification.
• Experience in geriatric rehabilitation or skilled nursing facility setting preferred.
• Ability to travel as assigned.
• Eligible to work in the United States without sponsorship.

Responsibilities

• Assess residents’ physical therapy needs and develop individualized treatment plans.
• Implement PT interventions including therapeutic exercise, gait training, balance, and mobility training.
• Reassess progress, modify goals as needed, and document outcomes.
• Administer modalities and assistive devices as appropriate; monitor safety and response.
• Maintain accurate documentation and communicate with an interdisciplinary team.
• Educate residents and families on home programs, safety, and prevention strategies; support discharge planning.
