Job summary

Affinity Home Health Care Inc. seeks a Travel Physical Therapist (PT) to assess patient movement in skilled nursing settings and implement individualized care plans. You will use exercises, manual therapy, and equipment to improve mobility and independence while collaborating with the care team.

The role requires a DPT, active state licensure, and BLS. Prior travel experience and rehab Optima familiarity are preferred.

Qualifications
