Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient conditions and educate patients and families in therapy methods.
• Develop care plans for physical therapy patients.
• Provide skilled physical therapy services per physician orders.
• Assess needs, plan, evaluate and modify care to meet therapy goals.
• Collaborate with multidisciplinary team to achieve patient goals.
