Job summary

Continuum Therapy Partners, LLC is seeking a traveling Physical Therapist to join Marshfield Health Services in Marshfield, Wisconsin for a 13-week, full-time assignment. You will provide skilled PT services, evaluate and treat patients, and contribute to discharge planning and functional outcomes.

This opportunity offers 35 hours per week with a focus on quality care in a post-acute setting. The network provides direct support, strong onboarding, and a clear communication channel with facility

Qualifications

• Active or eligible Wisconsin Physical Therapist / PT license.
• Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred.
• Travel experience preferred, but not required for the right clinician.
• Net Health experience is a plus.
• Strong documentation, communication, and team collaboration skills.
• Ability to start quickly and complete required onboarding before assignment start.

• Provide skilled Physical Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations.
• Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.
• Develop and update individualized treatment plans based on patient needs, goals, and progress.
• Collaborate with rehab leadership, nursing, facility teams, families, and care partners.
• Support discharge planning, patient education, and functional outcomes.
• Step into the assignment with professionalism, flexibility, and a patient-centered approach.

Continuum Therapy Partners is seeking a Physical Therapist / PT for a travel therapy assignment at Marshfield Health Services in Marshfield, Wisconsin.

This is a full-time travel opportunity within the Continuum Therapy Partners network. Continuum is the therapy provider supporting the facility directly, which gives travelers clearer communication, better insight into the assignment, and direct support from a team that understands the clinical environment.
