Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Monday to Friday, Days Hours per Shift: 32 Experience: New graduates accepted; SNF experience highly preferred License: Valid state license required Certifications: N/A Must-Have: Complete prescreen, resume, and license verification must be attached upon submission Description: Make sure the prescreen is filled out and attached upon submitting the candidate. Resume and license verification also need to be attached. If these items are not completed and attached, the candidate will be declined. Candidates must complete the credentialing checklist prior to submission. Required checks include: - State license verification - Resume - References (3) - County criminal background (7 years for all counties lived and worked) - Federal criminal background (7 years or more) - Multi-state criminal search (7 years or more) - Nationwide sex offender search (7 years or more) - HHS/OIG search - GSA search - Social security number trace - One-step PPD or Quantiferon or proof of positive PPD and chest X-ray - SAM search - Massachusetts background check - 8 hours of dementia training
