HiredFirst is seeking a Travel Physical Therapist for an outpatient assignment in Ukiah, CA. Evaluate and treat patients in an outpatient rehabilitation setting, develop individualized plans of care, provide therapeutic exercise and functional interventions, and document progress toward goals.

Assignment is 13 weeks with option to extend; 8-hour day shift, 5x8s, no weekends. Travel pay ranges from $2,582.60 to $2,869.56 weekly, with benefits including medical/dental/vision and recruiter support.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license required.
• Current BLS/CPR certification as required by the assignment.
• Recent physical therapy experience appropriate to the assigned setting.
• Strong assessment, treatment planning, documentation, communication, and patient-care skills.
• Ability to work the listed shift and schedule.

Responsibilities

• Perform physical therapy evaluations and establish individualized treatment plans and functional goals.
• Provide therapeutic exercise, mobility training, functional activities, and other appropriate interventions.
• Monitor patient response to treatment and modify the plan of care as needed.
• Educate patients, families, and caregivers regarding exercises, safety, mobility, and continued care.
• Complete accurate and timely clinical documentation and collaborate with the interdisciplinary care team.
