Alpha Home Health and Hospice is seeking an Outpatient Physical Therapist to provide therapy services to diverse patient populations in Georgia. You will evaluate patients, develop personalized rehabilitation plans, and monitor progress to improve mobility and function.

Responsibilities include collaborating with healthcare teams, delivering evidence-based treatments, and documenting progress to ensure quality care. SSN/DOB credentialing information is required for employment.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Active Georgia PT license (or eligible for compact license).
• Outpatient therapy experience preferred.
• Ability to provide references.
• Must have SSN and DOB for credentialing.
