Healthforce is seeking a Physical Therapist for a long-term care facility in Miami Gardens, FL. The assignment starts 11/02/2026 and runs 13 weeks, with a guaranteed 35 hours per week.

Shifts are five 8.5-hour daytime days, 08:00–16:30, and the role includes evaluations, treatment planning, and collaboration with an interdisciplinary team.

This travel assignment offers a weekly pay range of $2,223 to $2,337 and requires strong PT skills and documentation in a skilled nursing setting.

Qualifications

• Must have Physical Therapy licensure and be able to work in a long-term care setting.
• Strong documentation skills and ability to collaborate with interdisciplinary teams.
• Proficient in evaluating residents and developing plans of care.

Responsabilités

• Conduct comprehensive physical therapy evaluations for residents in a skilled nursing setting.
• Develop and implement individualized plans of care to improve mobility, strength, balance, and independence.
• Facilitate therapeutic interventions and monitor progress toward goals; reassess and adjust plans as needed.
• Document evaluations, treatment notes, goals, and progress per policy.
• Collaborate with an interdisciplinary care team and participate in discharge planning.
