Proforma Health Inc is seeking a Travel Physical Therapist to provide skilled services in a Skilled Nursing Facility. You will assess patient needs, develop and implement treatment plans, and deliver therapy in a patient-focused setting.

Qualifications

• Minimum 1 year of clinical PT experience.
• Valid PT license (including compact licensure accepted).
• BLS certification required.
• At least one professional reference.

Responsibilities

• Provide skilled physical therapy services and assess patient needs.
• Develop treatment plans and deliver therapy in a patient-centered environment.
• Work 40 hours weekly, with flexibility to float between Cedar Rapids and Keystone; maintain documentation.
