Qualifications

• Current State License as a Physical Therapist.
• Proof of CPR certification.
• Current driver’s license is required.
• 1–2 years experience as a Physical Therapist in an acute care or rehabilitation setting.
• Previous home health care experience.

Responsibilities

• Perform clinical assessments to identify patient needs and care appropriateness.
• Develop, implement, and evaluate patient care plans based on referrals and assessments.
• Provide treatment for neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families in transfer techniques, equipment use, and procedures.
• Collaborate with interdisciplinary team to ensure comprehensive care.
