Job summary

Elara Caring seeks a Physical Therapist to deliver high-quality, home-based patient care across the branch/office coverage area. You will assess needs, develop and implement plans of care, and collaborate with a team to ensure optimal patient outcomes.

You may travel up to 50% and will report to the Branch Director/Clinical Manager. APTA-approved program graduate with a current PT license is required, along with at least one year in a healthcare facility.

Qualifications

• APTA-approved physical therapy program graduate
• Current, unrestricted PT license valid for the state of work
• Minimum one year of experience in a healthcare facility
• Able and willing to travel within branch/office coverage area
• Must have a dependable vehicle, valid driver’s license, and current auto insurance
• Must be able and willing to travel 50%
• Ability to frequently lift, push, pull, and support up to 50 pounds, including transferring patients and moving equipment

Responsibilities

• Provides direct patient care and evaluates functional needs and outcomes
• Reviews patients’ functional needs and adjusts care plans to achieve optimal outcomes
• Designs and implements a plan of care based on thorough assessment
• Conducts therapy within defined standards of care
• Directs support staff to achieve patients’ functional goals
• Documents evaluations, treatment goals, plans, and maintains prescriptions
• Provides leadership to staff and promotes physical therapy in the community
• Handles physically demanding, high-stress environment
• Handles 50-pound lifting and transfers
• Possible irregular hours including call hours as applicable
