Job summary

Elara Caring is seeking a Physical Therapist to join our team in the home-care setting. You will provide direct patient care, evaluate functional needs, and design individualized plans of care for patients in their homes.

The role requires graduation from an APTA-approved program, an active PT license in the working state, and a willingness to travel up to 50% within the branch/office area. A dependable vehicle, DL, and insurance are required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area (50%).
• Must have dependable vehicle, valid driver’s license and auto insurance.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients, incorporating participation by significant others.
• Documents evaluations, treatment goals, and plans; maintains prescriptions and signatures.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Provides direction and leadership to staff and promotes physical therapy in the community.
