American Traveler is seeking a Home Health Physical Therapist with 1+ year of experience and AZ licensure to provide in-home patient care, performing evaluation, treatment, and documentation for home health PT patients. Day shift, 4x10 hours (7:00 AM–5:30 PM).

You may drive 30–75 miles per day, occasionally up to 150 miles, with mileage reimbursed; positions may require travel pairs. BLS certification and an AZ PT license are required; Meditech charting experience is highly preferred.

Requirements

• Minimum 1 year of experience required
• Home health or outpatient PT experience required
• Current PT license in AZ required (must be in hand at time of consideration); license required if applicable
• BLS certification required
• Level one fingerprint card required
• Experience with OASIS or home health not required

Responsibilities

• Home Health PT position treating patients in their homes and outpatient clinics
• Day shift schedule, 4x10 hours, 7:00 AM-5:30 PM
• Patient diagnoses include CVA, heart failure, DMII, s/p amputation, Parkinson's, paraplegia, and dementia
• No on‑call requirements
• Expect to drive 30-75 miles per day, occasionally up to 150 miles, with mileage reimbursed at the IRS standard rate
• Meditech charting system experience highly preferred
• Main responsibilities include evaluation, treatment, and documentation of home health PT patients
• Holiday coverage is required with minimal RTO requested
• First‑time travelers accepted
• Travel pairs accepted
• Candidates must live outside a 100-mile radius of the facility; local candidates not accepted
• Former permanent employees must wait 12 months before returning as travelers
