Qualifications

• Degree from an accredited Physical Therapy Program.
• Active clinician license or compact privileges in the state where services will be provided prior to beginning assignments.

Responsibilities

• Manage and monitor full patient cycle: perform an initial evaluation, establish a treatment plan, progress treatment and goals, communicate to physician, discharge, and follow-up.
• Accompany patients during their physician visits.
• Mentor new clinic staff.
• Complete patient documentation in a timely manner including initial evaluations, progress notes, discharge summaries, daily SOAP notes, functional status reports and insurance/plan of care forms.
