Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Fargo, North Dakota. This SNF/LTC role requires an experienced or newly graduated PT to join the team for a 13-week travel assignment with 40 hours per week.

Responsibilities include developing care plans, providing skilled PT services, and collaborating with a multi-disciplinary team to meet patient goals.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Current state license as a Registered Physical Therapist or actively pursuing licensure.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Develop and implement patient treatment plans in collaboration with supervising physicians.
• Deliver skilled PT interventions according to physician orders.
• Assess patient progress and modify plans as needed to meet goals.
• Work with multi-disciplinary team to coordinate care and goals.
