Northshore Home Health Care is seeking a Travel Skilled Nursing Facility (SNF) Physical Therapy Assistant (PTA) to support patients across diverse facilities. You will work under the supervision of a licensed Physical Therapist to help improve mobility and well-being in skilled nursing or long-term care settings.

Required are an Associate degree in Physical Therapy, an active state license, and BLS certification if required. Prior experience in SNF or long-term care is preferred.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active State License required.
• BLS Certification may be required.
• Experience in skilled nursing or long-term care settings.

Responsibilities

• Provide 8-hour shifts with no floating required.
• Support patient recovery and mobility.
• Collaborate with licensed Physical Therapists to develop and implement treatment plans.
