Details

• Home health setting covering assigned territories in Contra Costa County, including areas such as Walnut Creek, Danville, Concord, Antioch, and surrounding communities
• Day shift schedule with possible weekend coverage as needed
• Additional travel outside the assigned area may be required to maintain caseload
• Must be willing to see COVID-positive and COVID PUI patients if applicable

Requirements

• Current CA Physical Therapist license required if applicable
• Minimum 1 year of experience required
• BLS certification required
• Valid driver's license, auto insurance, and auto registration required
• Required documentation includes TB test (annual), MMR, Varicella, Hepatitis B, Tdap (within 10 years), Flu vaccination (annual), drug screen, and physical exam
