• Provide gait training, balance training, therapeutic exercise, mobility training, and fall prevention.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Communicate patient progress and condition changes to the interdisciplinary care team.

• Collaborate with RNs, PTAs, OTs, STs, and agency leadership on care coordination
• Ensure documentation complies with Medicare Conditions of Participation and HIPAA requirements
