Job Description & Requirements Specialty : Physical Therapist Discipline : Therapy Start Date : 06 / 26 / 2025 Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Unit Details : 8 hour days 40 hours per week No weekends Majority of Patients : general ortho and post operative Job Summary : Join a collaborative, patient-centered team at CMHP, providing skilled therapeutic interventions across a continuum of care including outpatient, inpatient, pediatric, and off-site settings. As a Physical Therapist, you will evaluate, plan, and implement personalized therapy programs, delivering evidence-based care aligned with the standards of the profession and the laws of the State of Ohio. Key Responsibilities : Perform thorough patient assessments and develop individualized treatment plans. Administer therapeutic interventions tailored to patient age, diagnosis, and goals. Collaborate with patients, families, and multidisciplinary teams to optimize outcomes. Educate patients and caregivers, ensuring safe and effective care transitions. Maintain accurate documentation and manage pre-authorization requirements. Participate in departmental quality improvement initiatives and clinical education programs. Provide services in clinical and community-based settings. Qualifications : Bachelor's degree in Physical Therapy (Doctorate preferred). Active Physical Therapy License in good standing. Current CPR certification. Specialty certifications (e.g., OCS, SCS, NDT, Lymphedema, Women's Health) preferred. Commitment to ongoing professional development and collaborative care.
