Responsibilities

• Assess referred patients needing rehabilitation and provide direct patient care.
• Develop and administer a treatment plan with physician-directed goals.
• Document patient progress and educate patients and families in therapy methods.
• Create a plan of care for each patient and adjust as needed.
• Provide skilled therapy interventions per physician orders.
• Assess and modify care to meet therapy goals.
• Collaborate with all disciplines to set team goals.

Requirements

Duties & Responsibilities

• Assessment of referred patients requiring rehabilitation services, including providing direct patient care to assess their medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients, document and record the patient's condition, and educate patients and families in an appropriate physical therapy method.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
