Centra Healthcare Allied is seeking a Travel Physical Therapist for a 13-week contract in Ashland, Wisconsin. The role is a SNF/LTC setting with a 40-hour week and dayshift.

Requirements include graduation from an accredited PT program, active or pending state licensure, and CPR certification. You will develop and implement care plans, provide skilled PT services, and collaborate with the care team.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Current state license as a Registered Physical Therapist or actively pursuing licensure.
• Current CPR certification required.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services according to physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to achieve patient goals.
