Job summary

Healthforce is seeking a Physical Therapist for a 13-week travel assignment in Clearlake, CA. The role offers a day shift (Mon–Fri, 5x8) with 40 hours per week and guaranteed hours.

This hospital-based setting provides patient evaluations, plan development, and collaboration with the care team. The ideal candidate holds a DPT or foreign equivalent and current CA licensure eligibility, with HCHB/OASIS/EPIC experience and at least two years of PT practice, including home health when possible.

Qualifications

• DPT degree or foreign equivalent and licensed in applicable states.
• Current physical therapy licensure and California eligibility for this assignment.
• CPR/BLS certified; additional variants accepted per policy.
• Valid driver’s license; must be at least 21.
• Experience with HCHB, OASIS, and EPIC; home health experience preferred.

Responsibilities

• Evaluate patients and create plans to restore physical function.
• Apply evidence-based PT interventions and adjust plans as needed.
• Document assessments, plans, progress notes, and discharges timely.
• Collaborate with care team and co-lead patient groups as appropriate.
• Supervise therapy assistants and communicate discharge needs.
