Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care.
• Develop and administer a treatment plan with goals per physician orders.
• Set realistic goals, document patient progress, and educate patients and families.
• Collaborate with all disciplines to plan and evaluate patient care goals.
