As part of a dedicated clinical team, you’ll evaluate and treat patients, develop personalized plans, and support discharge planning while adhering to safety and regulatory standards.

Qualifications

• Valid Physical Therapy license in compliance with state regulations.
• Current BLS certification (AHA/ARC).

Responsibilities

• Evaluate patients’ physical conditions using tests and history to identify needs.
• Develop personalized rehabilitation programs based on assessment findings.
• Implement treatment plans to improve mobility and reduce pain.
• Operate and maintain PT equipment safely and effectively.
• Document evaluations, interventions, and outcomes in the medical record.
• Collaborate with physicians and other professionals to optimize care.
• Educate patients on home exercise programs and self-management techniques.
• Maintain a safe environment with infection control and safety standards.
• Engage in quality assurance and professional development activities.
• Assist with discharge planning for smooth transitions to home or community settings.
