The position requires Cerner EMR experience, active AZ PT license, and BLS certification, with a 13-week contract and 40 hours per week. Weekend work is required.

Qualifications

Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs and goals.
• Provide interventions to improve mobility, strength, balance, and function.
• Help patients reduce pain and regain independence in daily activities.
• Monitor progress and modify plans as needed.
• Educate patients and families on exercises and strategies for recovery.
• Collaborate with physicians, nurses, therapists, and other team members.
• Maintain accurate and timely clinical documentation.
• Float to Acute Care as needed.

Position: Physical Therapist (PT)

Contract: 13 Weeks

Guaranteed Hours: 40 Hours/Week

Bill Rate: $85/hr

Position Overview

As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide evidence-based interventions designed to promote safe movement, recovery, and long-term independence

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs and goals.
• Provide therapeutic interventions to improve mobility, strength, balance, and function.
• Help patients reduce pain and regain independence in daily activities.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and families on exercises, mobility, and strategies for continued recovery.
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
• Float to Acute Care as needed.

Schedule

• Monday – Friday: 8:00 AM – 4:30 PM
• 40 hours/week
• Weekend Requirement: 2 weekends
• Weekend scheduling may be Sunday–Thursday or Tuesday–Saturday, based on facility needs.

Required Qualifications

• Current, active Arizona Physical Therapist (PT) License
• BLS Certification
• Cerner EMR experience – Required
• Recent experience in: Inpatient Rehabilitation
• Recent experience in: Skilled Nursing (SNU)
• Recent experience in: Acute Care
• Strong clinical assessment, communication, and documentation skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
