Qualifications

• Doctor of Pharmacy from ACPE-accredited program.
• Current pharmacist license to practice in the United States and state-specific licensure as required.
• Board Certification in Pharmacotherapy (BCPS) or equivalent; preferred.
• Immunization certification in accordance with state law.

Responsibilities

• Review and verify prescription orders for accuracy, safety, and appropriate dosing.
• Counsel patients and caregivers on medication use, dosing, side effects, and adherence.
• Prepare, dispense, and confirm medications per policy while ensuring availability and accurate labeling.
• Monitor for adverse drug events and drug interactions, escalating concerns to the care team as needed.
• Collaborate with prescribers and other clinicians to optimize therapy plans and address drug shortages or shortages.
• Document interventions and medication histories in the electronic health record to maintain a complete record of care.
