Responsibilities

Ensure accurate dispensing of medications and maintain patient safety on shift.

Counsel patients on medication usage, dosing, and potential side effects.

Document and report adverse drug events and near-misses promptly.

Adhere to regulatory standards, compounding protocols, and quality metrics.

Produce accurate medication profiles and maintain patient privacy per HIPAA/PHI guidelines.

Maintain precise medication inventory and manage replenishment orders.

Collaborate with healthcare teams to optimize pharmacotherapy plans.

Provide on-site support during day shifts with a 7-on/7-off schedule as assigned.

Engage in continuous professional development and pursue merit-based growth.

Utilize data and pipelines to track performance and target achievement.

Actively participate in team meetings and contribute to process improvement initiatives.

Experience/Qualifications

Active pharmacist license in the applicable state with no disciplinary actions.

Hospital/consultant pharmacy experience in dispensing and patient counseling.

Proficiency in HIPAA/PHI compliance and patient data privacy practices.

Strong attention to medication safety, adverse events reporting, and near-miss documentation.

Experience with inventory management and medication procurement processes.

Ability to work day shift on a 7-on/7-off schedule with on-site presence.

Mechanical/clinical accuracy in building and maintaining patient medication profiles.

Collaboration skills to support multidisciplinary teams and pharmacotherapy optimization.

Data-driven mindset with ability to use pipelines to track performance and targets.
