Jackson PharmacyPros is seeking a licensed travel pharmacist for a 13-week assignment in Danville, Kentucky. You will deliver clinical expertise to safely distribute medications while supporting cost-effective care for patients across diverse settings.

The role includes medication reconciliation for discharge, discharge counseling, and collaboration with the healthcare team on antimicrobial stewardship, pharmacokinetic and anticoagulation dosing, with Epic systems experience preferred.

• Medication Reconciliation for discharge medication list.
• Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy.
• Communication skills for discharge counseling with patients/families.
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations and knowledge of UTI, pneumonia, SSTI, etc.
• Pharmacokinetic dosing management/monitoring.
• Anticoagulation dosing management/monitoring.
