• Schedule: Nights, 10 hours per shift, with a minimum of 40 hours per week
• Required Qualifications: Virginia State Pharmacist license, hospital pharmacy experience of at least 2 years, proficiency with Meditech, Vigilanz, Dose Edge, Pyxis, C2 safe, IV verification, medication verification, IV compounding, and dose calculation protocols, experience with Vancomycin AUC dosing, Aminoglycoside dosing, renal dosing, warfarin, enoxaparin, electrolyte protocols, heparin, and Direct Oral Anticoagulants (DOACs).
• Preferred Qualifications: Experience with complex inpatient medication therapies, advanced skills in pharmacokinetic dosing and renal adjustments.
• Additional Information: Experience with hospital electronic documentation systems, knowledge of unit-specific policies, and the capability to manage complex patients across multiple critical care units are essential. Resources such as Sanford Guide, Clinical Key, Uptodate, Vanc AUC calculator, Neofax, Briggs, and Lexicomp are utilized regularly.
