The ideal candidate will hold a Nevada Pharmacist license (active or temporary) and have at least 1 year of pharmacist experience.

• Minimum of 1 year of practice as a pharmacist.

Aufgaben

• Deliver face-to-face, telephonic, or virtual pharmaceutical care services to patients.
• Support medication reconciliation, disease management, and drug therapy assessments.
• Conduct clinical reviews, medication monitoring, and provide disease state counseling.
• Document all patient interactions in the EMR (EPIC).
• Assist with medication profile management and drug utilization data analysis.
• Participate in interdisciplinary rounds and quality improvement initiatives.
