Details

• Outpatient setting located within a multi-service health park offering holistic, family-focused care
• Monday-Friday day shifts, 8-hour shifts (8am-4:30pm or 8:30am-5pm)
• No weekend or on-call requirements

Requirements

• Active PT license required (license required if applicable)
• Active Michigan PT license verified through LARA and/or Nursys required
• Current AHA BLS certification required
• PT outpatient experience required
• Acute care experience highly preferred
• Skills checklist pertinent to PT specialty, current within 1 year, required
• Two completed supervisory reference forms from within the last two years required

Additional Information

• Responsibilities include patient evaluation, treatment planning, and coordination of care with physicians and support staff
• Delegates and oversees Physical Therapist Assistants and Aides
• Develops and updates individualized treatment plans based on assessment findings
• Educates patients and families on conditions, treatment rationale, and care alternatives
• Dress code includes khaki pants and navy blue logo wear
• Candidates must reside outside a 50-mile radius of the facility to be considered
• No more than 5 days of requested time off allowed during the assignment
