Job Details

• Position Description: Provides outpatient physical therapy evaluations and treatments, develops and modifies treatment plans based on patient needs, goals, and physician directives. Responsible for delegation and supervision of therapy services for assigned patients, ensuring functional treatment programs that promote effective community reintegration. Serves as an educator to patients, families, students, and colleagues, adhering to professional standards and organizational core values.
• Certifications, Education, and Experience: Degree in Physical Therapy from an accredited school, 6 months to 1+ year of outpatient clinical experience, CPR certification, active applicable state license, and 1 year of experience preferred.
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Additional Responsibilities: Performs comprehensive patient evaluations, creates individualized treatment plans, completes accurate medical record documentation, and adheres to all safety guidelines emphasizing patient and employee safety. Participates in student training, actively pursues professional development, and complies with regulatory standards and licensure requirements.
