Job summary

Healthforce seeks an Occupational Therapist, General for a travel assignment in Waterville, Maine. The role focuses on delivering general OT services in a skilled nursing facility, collaborating with an interdisciplinary team to support rehabilitation goals and safe discharge planning.

The assignment runs 13 weeks starting 11/09/2026 with guaranteed 35 hours per week and a shift from 08:00 to 16:30, 5 days a week. Active OT license and ACOTE-accredited education are required.

Qualifications

• Active OT license (state of practice).
• Graduation from an accredited OT program (Master's or Doctorate).
• Ability to work in a skilled nursing facility.
• Prior experience in rehabilitation or long-term care preferred.
• Willingness to participate in travel assignment in Waterville, ME.

Responsibilities

• Conduct OT evaluations and develop treatment plans to improve ADLs, mobility, balance, and safety.
• Implement evidence-based OT interventions and adapt plans based on progress and regulatory requirements.
• Document progress and outcomes in the medical record, ensuring accuracy and confidentiality.
• Collaborate with an interdisciplinary team to align therapy goals with overall care plans.
• Educate residents and families on home exercise programs, adaptive equipment, and discharge planning.
• Monitor progress and participate in discharge planning and transitions.
