• Master's or Doctorate in Occupational Therapy required or eligible.
• Active state OT license or pending licensure.
• SNF experience preferred but not required.

Responsibilities

• Evaluate and treat patients with functional, cognitive and physical impairments.
• Address ADLs, IADLs, upper-extremity rehab and adaptive strategies.
• Provide 1:1 patient-centered care.
• Collaborate with PT, SLP, nursing and case management.
• Complete accurate documentation and discharge planning.
