Job summary

American Traveler, LLC. is seeking an Occupational Therapist for a Home Health position in Hermiston, OR. The role requires 3+ years of experience and a personal vehicle for in-home visits.

Responsibilities include home visits, coordinating with the interdisciplinary team, evaluating functional status, and documenting care. Weekend coverage and CRS AE training may be required for longer assignments.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Eligible for the National Registration Examination of the American Occupational Therapy Association.
• Minimum 3 years of experience required.
• Current BLS certification required.
• Valid driver's license and proof of auto insurance required.
• License required if applicable.

Responsibilities

• Home Health setting providing in-home patient care.
• Day shift, Monday through Friday, 8:00 AM to 4:30 PM.
• 5 shifts per week, 8 hours per shift.
• Weekend rotation required, including coverage on New Year’s Day.
• Candidate must have own vehicle for travel to patient homes.
• Serve as an integral member of the interdisciplinary home health team.
• Evaluate patient functional status and implement physician's plan of care.
• Complete assessments and reassessments based on objective tests and measurements.
• Communicate with team members to ensure coordinated patient care.
• Provide education and direction to patients and coworkers.
• Document all communications, coordination, and treatment activities accurately.
• Adapt to emergency situations and changes in patient condition.
• Work with elderly populations and long-term care and hospice patients.
• CRASE training required if assignment extends beyond 6 months.
