Job summary

Healthforce Healthcare LLC is seeking an Occupational Therapist for a travel assignment in Canaan, Connecticut. The position runs for 13 weeks starting 12/01/2026 with a 35-hour work week and shifts from 08:00 to 16:30 in a long-term care nursing home.

The role requires an active CT OT license, a degree from an accredited OT program, and preference for geriatric/long-term care experience. Travel-based assignment with 24/7 support is provided.

Qualifications

• Active Occupational Therapist license in Connecticut is required.
• Experience in geriatric or long-term care settings is preferred.
• Strong communication, documentation, and collaboration skills are essential.

Responsibilities

• Conduct OT evaluations and develop individualized treatment plans for residents.
• Provide and supervise OT services to improve ADLs, mobility, and cognition.
• Implement therapy sessions, monitor progress, and adjust plans based on resident response.
• Document progress in medical records and communicate findings with the multidisciplinary team.
• Educate residents and families on therapy goals, home programs, and safety.
