Job summary

HealthForce Healthcare seeks a licensed Occupational Therapist for a travel assignment in Tacoma, WA. The 13-week contract starts 11/23/2026, with guaranteed 35 hours per week and a five 8-hour shift from 08:00 to 16:30.

Weekly pay ranges from $2,498 to $2,597, and the setting is a skilled nursing facility. Responsibilities include comprehensive assessments, goal-directed interventions, and interdisciplinary collaboration to optimize resident outcomes.

Qualifications

• Current, valid Occupational Therapist license to practice in Washington state or eligibility to obtain licensure.
• Degree in Occupational Therapy from an accredited program.

Responsibilities

• Perform comprehensive OT assessments and establish individualized treatment goals for residents with functional limitations related to ADLs, mobility, and cognitive/perceptual deficits.
• Plan, implement, and monitor evidence-based OT interventions to improve independence in ADLs, mobility, gait and balance, upper-extremity function, and cognitive skills.
• Document evaluations, progress notes, treatment plans, and discharge summaries in accordance with regulatory and facility standards; maintain accurate, timely records.
• Collaborate with an interdisciplinary team including nursing, physical therapy, speech therapy, social work, and physicians to coordinate care and optimize outcomes; participate in care planning meetings.
• Educate residents and families on therapy goals, home exercise programs, safety strategies, fall prevention, and adaptations to living environments; provide caregiver training as needed.
• Ensure compliance with risk management, infection control, and safety guidelines; monitor for adverse events and adjust interventions accordingly; participate in quality improvement initiatives.
