Job summary

Proforma Health Inc. is seeking a Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility setting in New Jersey. The role focuses on aiding patients to regain daily living skills under supervision of a Licensed Occupational Therapist.

You will assist with treatment plans, document patient progress, and work with geriatric populations using SNF equipment. A NJ COTA license and BLS are required; SNF experience is preferred.

Qualifications

• Active New Jersey COTA license required.
• Associate degree in Occupational Therapy from an accredited program.
• BLS certification is preferred or required.
• Experience in SNF settings preferred.
• Ability to follow treatment plans and document progress.
• Familiarity with geriatric care and SNF equipment.

Responsibilities

• Provide patient care during day shifts (08:30–16:30) on a 5x8 schedule.
• Work 40 hours/week with guaranteed 36-40 hours.
• Follow supervision by a licensed Occupational Therapist and facility policies.
• Assist patients in regaining daily living skills and document progress.

• work with geriatric populations and SNF equipment such as adaptive seating, orthotics, and ADL training tools.

Qualifications & Skills:

• Associate degree in Occupational Therapy from an accredited institution
• active New Jersey COTA license
• BLS certification
• preferred experience in SNF settings
• ability to follow treatment plans and document progress
• familiarity with geriatric care and SNF equipment

Key Duties:
