Job summary

The Mental Health Association of Rochester is seeking a Travel Home Health Occupational Therapist to provide therapy across assigned counties, assess and treat patients, and document progress in HCHB. You will coordinate care with the care team to improve daily functioning.

Ideal candidates hold OT licensure with a degree (Bachelor/Master/Doctoral) and at least one year of home health experience; BLS may be required.

• Active OT license
• At least 1 year home health experience
• BLS certification may be required

Travel Home Health Occupational Therapist

Responsibilities

Provide therapy services to patients with injuries, illnesses, or disabilities to improve their daily skills. Cover areas including Fairfield, Perry, and Hocking counties, with documentation in HCHB. Ensure patient care, maintain licenses, and manage documentation requirements.

Qualifications & Skills

Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited program, active OT license, and at least one year of home health experience. BLS certification may be required. Strong communication and documentation skills.

Key Duties

Deliver 8-hour shifts, assess and treat patients, document progress, and coordinate care. Mileage reimbursement between visits and other travel-related benefits are provided.
